We are currently running 2.6.4.951 version on our Octopus Deploy and planning to upgrade it to Version 3.
Is it possible to upgrade to 3.4.12 right the way or we need to upgrade to older version first.

You can upgrade to any 3.x version, and we would always suggest the latest. Be sure to plan out your upgrade using our guides (if you haven’t already!) http://docs.octopus.com/display/OD/Upgrading+from+Octopus+2.6
