OctoPack readme is incorrect regarding version number

I believe there’s a difference between what’s coded in https://github.com/OctopusDeploy/OctoPack/blob/master/source/OctoPack.Tasks/GetAssemblyVersionInfo.cs and what’s documented in the readme of OctoPack regarding how the package version is determined.

Would be better to fix the readme because the code is hard to figure out.
